Primary Care, from the comfort of home
Our Approach
At VPC, our clients receive in-person primary care and clinical support from the comfort of their own home. Our in-home services include primary care visits, well visits, urgent care response when needed, and 24/7 clinical support. We also coordinate in-home blood draws, EKGs and labs, and medication management. For over 10 years, we have been serving our communities in-person and have built deep connections with our patients and their caregivers.

How We Are Different
We are a physician-led interdisciplinary care team, including nurse practitioners, physician assistants, community health workers and medical assistants. Our providers speak multiple languages and deliver care in-person at home.
By specializing in caring for older adults, our focus is on meeting their specific needs, especially those who are homebound or have difficulty getting to and from a provider’s office. We partner with specialists who make house calls and laboratory and diagnostic imaging companies who can provide in-home testing when possible.
At VPC, care coordination is a high priority and core to the services we provide. We believe that a patient’s care network consists of not only the medical care team but also home health aides, family and other caregivers. And because we specialize in the care of older adults, managing multiple chronic conditions is central to what we do.
